Comments:

An unannounced renewal inspection was conducted today from 9:10am-10:00am with the provider on site. There were7 children in care (19 points) with 2 caregivers. A sample of 2 children's files and 2 caregiver files were reviewed. The children were engaged in the following: playing with toys, painting a tree and washing hands. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and caregivers were positive. The physical space, attendance records, drill record and the Emergency Preparedness Plan were reviewed. There were no medications on site. Areas of non-compliance are identified in the violation notice. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver names; list caregivers by positions only. Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website. Description:	Based on review of caregiver records and interview, the provider did not obtain a central registry report within 30 days of employment for one caregiver. Evidence: 1. Caregiver #3 (Date of hire 9/12/2023) had a central registry report dated 11/15/2023. 2. The provider stated it was sent on 9/29/2023 and then returned and resent again on 10/19/2023 for payment.
Plan of Correction:	Caregiver #3 did the processon time but the registration reports 11/15/2023.
Standard #:	8VAC20-800-60-B
Description:	Repeat Violation Based on review of two children's records, the provider did not maintain complete children's records. Evidence: 1. Child #1 (start date 9/08/2023) was missing one emergency contact name, address and telephone number.
Plan of Correction:	I filled this missing part.
Standard #:	8VAC20-800-170-A
Description:	Based on review of caregiver records, the provider did not obtain documentation of a tuberculosis test or screening for each caregiver at the time of hire. Evidence: 1. Caregiver #3 (Date of hire 9/12/2023) had a tuberculosis test or screening dated 9/16/2023.
Plan of Correction:	Caregiver #3 did the TB test 9/10/2023, the results dated 9/16/2023.
