A complaint was received in the licensing office on 12/4/2023 regarding staffing and supervision. On 12/5/2023 an on-site investigation was conducted. Interviews were conducted and records were reviewed. At the time of this investigation, the preponderance of evidence did not support the allegation and the complaint is deemed not valid, however additional violations were found during the investigation and can be viewed on the Violation Notice.

Standard #: 22.1-289.035-B-2 Complaint related: No Description: Based on record review and interview, the provider failed to ensure that each caregiver submitted to a fingerprinting background check prior to coming into contact with children.
