Comments:
An unannounced monitoring inspection was conducted October 30, 2023, as a follow-up to the issuance of a provisional license. The inspector was on site from approximately 1:20 pm until 3:00 pm. There were three children in care with the provider for a value of ten points. One child played on the floor with toys while two children were still napping. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four children?s records and two household member records were reviewed. The inspector interacted with the caregiver and children in care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.036-A
Description: Based on review of two household member's records and interview, the provider did not obtain a current central registry check every five years from the initial central registry check for each household member.

Evidence:

1. The most recent central registry check for household member #1 was dated 03/11/2018, exceeding five years.
2. The most recent central registry check for household member #2 was dated 03/06/2018, exceeding five years.
3. The provider had documentation of where an updated central registry check had been requested for household member #1 and #2, however the results of the central registry check had not been received and a follow-up to the status of the background check had not been conducted

Plan of Correction: Per the provider: I had my daughter send off the request but we didn't receive any results. I am working with PCPC for assistance and have resubmitted the request and will contact them if I do not receive results within 30 days. I will also keep all the documentation.

Standard #: 8VAC20-800-540-A
Description: Based on observation, the provider did not ensure Infant carrier seats, swings, strollers, feeding or activity tables, and high chairs were used according to the manufacturer?s instructions and when occupied by a child, a safety strap was securely fastened.

Evidence:

Child #4, 11 months in age, was observed sleeping in a bouncer without the safety strap fastened. The provider stated the seat belt was not fastened.

Plan of Correction: the provider secured the seatbelt while the inspector was on site. Per the provider: I will fasten the seat belt whenever children are in them. I was sitting next to the seat and had just gotten her to sleep.
