Discussion that the CPS request for Staff #3 was sent. LI gave contact number to Provider to follow-up after 30 days.
1.5 hours of annual training left for Staff #1, and 2.5 hours of annual training left for Staff #2. According to Provider, both certificates of completion were on their way. Please place on file for future review.
Provider is currently changing out all 3 smoke detectors on the lower level of her home. Existing detectors were still in place on upper floors of the home.

This monitoring inspection was initiated by licensing staff and concluded on 12/06/2023, between the hours of 9:30 am to 11:30 am. There were 12 children present (27 points), ranging from infant to preschool-age, with 3 caregivers supervising. The children were engaged in the following: large motor skills in playroom, colors, shapes, letters, learning, music, song, story time, bathroom breaks, diapering, hand washing, lunch, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 12 children records, and 3 staff records were reviewed.
Information gathered during the inspection determined that there was (1) non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the home.

Evidence -
1. On the date of inspection (12/05/2023), orientation documentation training for Staff #3 was not available for review.
2. The listed start date for Staff #3 was listed as 11/06/2023.Plan of Correction: An orientation training for Staff #3 will be completed and placed on file.
