Based on observation, the provider did not ensure areas outside the family day home were maintained in a safe condition.
Evidence:
The top plank on the climbing wall on the play structure has a crack exposing a screw head. The crack is splintering and wide enough to pose a finger entrapment hazard.Plan of Correction: We are going to toss the play set.
