Comments:

A monitoring inspection was conducted on 12/07/2023. There were a total of 3 children (8 points) observed with 3 caregivers present. The children were engaged in the following: playing with puzzles and transitioned to naptime. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, diapering, medication, emergencies and nutrition. A total of 3 child records and 4 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today in order for it to be included on the website. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description:	Based on a review of documentation, the provider did not ensure emergency evacuation procedures were practiced monthly. Evidence: The last documented emergency evacuation drill was on 09/15/2023.
Plan of Correction:	I will update and do required drills.
Standard #:	8VAC20-800-210-A
Description:	REPEAT VIOLATION Based on record review and provider statement, staff have not completed sixteen hours of training annually in areas relevant to their job responsibilities. Evidence: Staff 1, 2, 3 records do not contain documentation of annual training. Staff 1 stated that no annual training has been completed.

Description:	REPEAT VIOLATION Based on observation, the provider did not ensure areas outside the family day home were maintained in a safe condition. Evidence: The top plank on the climbing wall on the play structure has a crack exposing a screw head. The crack is splintering and wide enough to pose a finger entrapment hazard.
Plan of Correction:	We are going to toss the play set.

Description:	Based on observation and statement, the provider did not ensure a landline telephone was available and operable during the inspection. Evidence: There was no landline telephone available. The provider confirmed there was no landline telephone.
