Technical Assistance:
Provided consultation on standard:
1) 560A: monitoring sleeping infants, safe sleep, and ensuring the direct supervision of all children; blankets over heads prevent direct supervision of children and their breathing
2) 920: the menu shall be: dated, specifically list what foods are to be served, list any substituted foods, and be kept on file for one week; parents should know ahead of time exactly what will be served, and afterwards, what was served

Comments:
An announced monitoring inspection was conducted from 1:45 - 2:45 pm with the provider. There were seven children in care, ranging from eight months to five years of age, for a total of 15 points. Children were observed napping and one infant was observed playing. Five child records and three caregiver/household member records were reviewed along with the written attendance record, emergency drill log, menu, and required posted information. The provider has current certification in CPR and First Aid, but is not Medication Administration Training (MAT) certified and cannot administer medication to children in care.

Standard #: 8VAC20-800-900-B
Description: Based on observation, it was determined that children were allowed to drink while lying down. Evidence: an empty bottle was observed in a crib with a sleeping infant.

Standard #: 8VAC20-800-940-A
Description: Based on observation, the home did not ensure that temperatures were maintained at or below 40 degrees F in the refrigerator and at or below 0 degrees in the freezer. Evidence: according to the thermometers, the temperature in the refrigerator was 45 degrees F and the temperature in the freezer was 3 degrees F.

Standard #: 8VAC20-800-100-A
Description: Based on review of five child records, the provider did not obtain documentation that each child has had a physical prior to the child's first attendance or within 30 days after the first day of attendance. Evidence: the record for child #1 (date of first attendance: 9/11/2023), child #2 (date of first attendance: 7/28/2021), child #3 (date of first attendance: 5/30/2023), child #4 (date of first attendance: 9/11/23) and child #5 (date of first attendance: 9/11/2023) did not have documentation of a physical exam.

Description: Based on observation, not all cribs in use had tight fitting sheets. Evidence: an infant was observed in a crib that had loose sheets. Loose crib sheets are a suffocation hazard and increase the risk of sudden unexplained infant death.
