Comments:
An announced initial inspection was conducted from 10:30 am - 11:30 am. There were no children in care with the provider. The family day home was observed for age/stage appropriate supplies, activities, and equipment for the ages of children requested for licensure. Background checks were reviewed for the provider and household members and were in compliance. The inspector reviewed compliance in the areas of administration, physical plant, staffing and programming, medication, special care and emergencies and nutrition.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: Based on observation and interview it was determined that the family day home did not have landline service or an operable landline telephone. Evidence: There was not a landline telephone or landline service in the family day home as required.

Plan of Correction: The provider will order landline service and purchase a landline telephone.
