A Fictitious name will be needed for the home to include the LLC name on the license. The form will need to filed with the VA SCC. It was sent with the monitoring notes to the provider.

Comments:
An unannounced monitoring visit was conducted. The provider was completing an on-line course for her CDA and had 3 assistants present with 6 children for fourteen points which is within ratios for supervision Observed children during lunch (chicken quesadilla, rice, cantaloupe and berries and milk), diapering and transition to rest time. Dedicated space in the basement with 2 large rooms, a separate sleeping space for cribs and restroom for children in care. No medication was reported to be on-site. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please be aware that the plan of correction must be provided within 5 days of receipt of the inspection notes if you want it displayed on the website.
