Technical Assistance:
Fictitious Name documentation required for renewal

Documentation of portability required to be printed out from OBI when a new assistants are hired.

Comments:
An unannounced renewal inspection was conducted today from 12:30 pm-1:10 pm. There were 7 children directly supervised by 3 staff. The home, 2 staff records, 4 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, reading books, getting ready to pick up friends from school and eating lunch/ drinking bottles Diapering procedures were also observed and or discussed. There was an ample supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-800-960-E
Description: Based on observation bottles were not labeled as required.
Evidence: 4 bottles were not labeled with child's name and date as required.

Plan of Correction: I will remind them to label when they get here.

Standard #: 8VAC20-800-120-B
Description: Based on record review required documentation was incomplete.
Evidence:
Staff A- record did not contain 2 or more refences as to character and reputation before employment.

Plan of Correction: I did them but didn't write it down. I will do again.

Standard #: 8VAC20-800-60-A
Description: Based on record review, child records were not available.

Evidence:
Child #1- did not have a record maintained by the provider. ( start date 10/31/2023)
Child #2- did not have a record maintained by the provider. ( start date 10/31/2023)

Plan of Correction: They just started today. I will get from mom at pick up.
