Comments:
A non-mandated monitoring inspection was conducted on December 7, 2023 from 11:45 A.M.-1:45 P.M. There were three children present for a total of ten points, ranging in ages from nine months to two years of age, with one caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and one caregiver records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-100-A
Description: Based on review of children records, the provider failed to obtain a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
1. Child #1 and #2's first day of attendance was 09/12/2023. There was not a physical examination in either of the children records.
2. The provider verified that she had not obtained the physical examinations.

Plan of Correction: Was confused between voluntarily registered programs and family day home standards. Will request physical from the parents. In the future will request physicals by the 30th date of enrollment.

Standard #: 8VAC20-800-60-A
Description: Based on a review of children records, the provider failed to ensure that all required information was in the children records.

Evidence:
1. Child #1's record did not contain the name of the child's medical insurance and policy number.
2. Child #2's record did not contain the following: address for the emergency contacts, written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately, and the name and policy number of the child's medical insurance.

Plan of Correction: Will contact the parents to obtain required information. For future enrollments will audit information to make sure all is complete before the child attends.

Description: Based on a review of children records, the provider failed to provide the provisions of the family day home's emergency preparedness and response plan.

Evidence:
1. Child #1's first date of enrollment was 09/12/2023. There was not documentation in the record that the parent had received the provisions of family day home's emergency preparedness and response plan.
2. Child #2's first date of enrollment was 09/12/2023. There was not documentation in the record that the parent had received the provisions of family day home's emergency preparedness and response plan.
3. The provider verified that the parents had not been provided in writing of the provisions of the family day home's emergency preparedness and response plan.

Plan of Correction: Will contact the parents to provide parents with written agreements and received signed copy. For future enrollments will audit information to make sure all is complete before the child attends.
