Technical Assistance:
Inspector recommended the provider seek additional training by attending the Department of Education Phase 2 training for Family Day home providers.

Comments:
A mandated renewal inspection was conducted on December 8,2 023 from 9:45am-11:00am. There were 10 children in care with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Children were observed playing with toys, eating snack and watching television. A total of 2 child records, 2 staff records and 2 household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-750-A
Description: Based on observation and record review, topical skin products did not meet all the requirements.

Evidence: 9 tubes of diaper ointment were not labeled with the child?s name. Children?s records did not include written parent authorization noting any unknown adverse reactions.

Plan of Correction: diaper ointment have been removed and notification to parents will be used as needed.

Standard #: 8VAC20-800-830-B
Description: Based on record review, shelter in place was not practiced twice a year.

Evidence: There was no documentation of a shelter in place drill in 2023.

Plan of Correction: yes we practice shelter in place just forgot to note last

Standard #: 8VAC20-800-890
Description: Based on observation, formula brought from home was not labeled in a way that identifies the owner.

Evidence: he formula for an infant was not labeled.

Plan of Correction: parent has been notified that needs to label bottles/formula

Description: Based on observation and record review, the menu was not dated and kept on file for one week at the family day home.

Evidence: The menu posted was dated for November.

Description: Based on observation, the drinks used for children in care were not kept in the refrigerator.

Evidence: Milk brought from home was stored in a lunch bag with a cold pack on the counter.

Plan of Correction: milk will be stored on refrigerator at all times.

Description: Based on observation, the bottles in the refrigerator were not labeled with the child?s full name and date.

Evidence: There were two infants in care and the bottles in the refrigerator were not labeled.

Plan of Correction: the parent has been notified to label bottles

Standard #: 8VAC20-800-210-A
Description: Based on record review, caregivers did not obtain a minimum of 16 hours of annual training.

Evidence: Caregiver 1 and 2 did not complete a total of 16 hours of annual training. Caregiver 1 and 2 have no hours of annual training.

Plan of Correction: Will do a better job for 2024. Will have 8 hours by MAy 2024

Standard #: 8VAC20-800-270-A
Description: Based on interview, it was determined that the firearm in the home was not stored in a locked container, compartment, or cabinet, and apart from the ammunition.

Evidence: Caregiver 2 stated that the firearm and ammunition were stored in a locked cabinet together.

Plan of Correction: I think this is an overreach regulation kids are cared on basement an impossible that they open a safe where my gun is.

Standard #: 8VAC20-800-430-A
Description: Based on observation, children under two years of age had access to stairs.

Evidence: There was no gate at the bottom of the stairs which children under two years old have access

Plan of Correction: a new stair gate has been purchased and installed

Standard #: 8VAC20-800-600-B
Description: Based upon observations, an infant spent more than 30 minutes of consecutive time during waking hours in a confining piece of equipment.

Evidence: During the time of the inspection, from 9:50am-1030am Child 1 (7 months) was placed in confined piece of equipment without an intervening time of at least one hour in between them. At about 9:50am, Child 1 was in a feeding table with no food, then a bouncer, then a crib and back to a feeding table until about 10:40am. Child was an a confined space for about 50 minutes.

Plan of Correction: Time in confined space will be closely monitored and no more than 30 mins as per regulation.

Standard #: 8VAC20-800-680-A
Description: Based on observation, the caregiver did not wash their hands with liquid soap and warm running water before feeding or helping children with feeding.

Evidence: Caregiver 1 did not wash their hands before serving snack to the children.

Plan of Correction: Caregivers will wash hands at all times.

Standard #: 8VAC20-800-680-B
Description: Based upon observations, the caregiver did not ensure that all children?s hands are washed with liquid soap and warm running water before and after eating.

Evidence: The children sat and were served snack without washing hands. When snack was over, Caregiver 1 did not wash all children?s hands after eating.

Plan of Correction: Will make sure all children wash hands

Standard #: 8VAC20-800-690-E
Description: Based on observation, soiled diapers and wipes were not disposed of in a leak-proof storage system that is either foot operated or used in such a way that neither the caregiver?s hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal.

Evidence: Caregiver 1 lifted the lid of the garbage can to dispose of diapers and wipes. The garbage can was not foot operated.

Plan of Correction: a new foot operated system has been purchased and already functioning
