Description: Based on records reviewed, there was no documentation that an adult household member has completed a TB screening every 2 years. Evidence: The TB test on-file for household member #3 was dated 5/28/21.

Standard #: 8VAC20-800-210-A
Description: Based on records reviewed and interview conducted, a caregiver did not obtain a minimum of 16 hours of annual training. Evidence: Documentation showed that Caregiver #1 obtained 4 of 16 hours of annual training and this was confirmed by the caregiver.
