Comments:

An unannounced monitoring inspection was conducted at 12:10pm. Observed 5 children totaling 19 points with 2 caregivers. Ratios and supervision were in compliance. Children were finishing lunch, washing hands and playing with toys during the inspection. Lunch served included chicken, mixed vegetables, bread, banana and milk. The daycare areas of the home were found to be clean and sufficiently supplied with toys and equipment for the children. Areas of non-compliance with standards and laws reviewed were found. Description:	Based on records reviewed, 2 of 5 children's records did not include documentation that the parents had reviewed emergency contact information and signed the form annually. Evidence: Child #2-The parents last signed the form July 2022.
Plan of Correction:	the parents signed the forms. [sic]
Standard #:	8VAC20-800-100-A
Description:	Based on records reviewed, 1 of 5 children's records did not include a physical exam within the first 30-days of attendance. Evidence: Child #5 began attending 6/20/23 and to-date there was no physical exam on-file.
Plan of Correction:	Informed the parent
Standard #:	8VAC20-800-60-B
Description:	Repeat violation. Based on records reviewed, 2 of 5 children's records did not include required information. Evidence: 1. Child #3-1 of 2 emergency contacts (name, address and phone) was left blank and the medical insurance number was left blank. 2. Child #4-The medical insurance number was left blank and the emergency medical authorization was not signed by the parent.

Description:	Based on records reviewed, 2 of 5 children's records did not include documentation of immunizations every 6 months until the age of 2 years old. Evidence: 1. Child #2-The immunizations on-file were dated September 2022 when the child was 8 months old. 2. Child #3-The immunizations on-file were dated February 2023 when the child was 6 months old.

Description:	Based on records reviewed, 0 of 2 caregivers completed a TB screening every 2 years. Evidence: The TB tests on-file for both Caregiver #1 and #2 were dated 5/30/21.
