Comments:

An unannounced monitoring inspection was conducted today between 12:20pm-1:25pm. There were 3 children present with the Provider for a total of 7 points within the supervision guidelines. The children were observed playing with toys and engaging in other activities. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, evacuation drill log, attendance and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation. Standard #:	22.1-289.035-A
Description:	Based on a review of Caregiver records, all employees did not undergo a background check in accordance with subsection B prior to employment and every five years thereafter. Evidence: 1. Caregiver #1 had Fingerprints background check results dated 5/23/2018, which were expired. 2. Caregiver #1 had Central Registry background check results dated 10/16/2015, which were expired.
Plan of Correction:	We will get new background checks done.
Standard #:	22.1-289.036-A
Description:	Based on a review of household member records, 2 out of 2 household members did not undergo a background check in accordance with subsection B prior to issuance of a license as a family day home and every five years thereafter. Evidence: 1. Household member #1 had Fingerprints background check results dated 5/23/2018, which were expired. 2. Household member #1 had Central Registry background check results dated 10/6/2015, which were expired. 3. Household member #2 had Fingerprints background check results dated 5/23/2018, which were expired. 2. Household member #2 had Central Registry background check results dated 10/6/2015, which were expired.
Plan of Correction:	We will get new background checks done.
Standard #:	8VAC20-800-100-A
Description:	Based on a review of children's records, 1 out of 3 children did not have a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. Child #1 (doe: 11/14/2022) did not have a physical examination on file at the time of the inspection.
Plan of Correction:	We will ask the parent to bring the physical.
Standard #:	8VAC20-800-180-A
Description:	Based on a review of caregiver and household member records, all caregivers and household members do not have a current tuberculosis screening every two years from the date of the first screening. Evidence: 1. Caregiver #1 has a TB screening dated 5/5/2017. 2. Household member #1 has a TB screening dated 3/6/2020. 3. Household member #2 has a TB screening dated 5/10/2018.
Plan of Correction:	We will get new TB screenings.
Standard #:	8VAC20-800-690-D
Description:	Based on observation, the diapering surface was not nonabsorbent. Evidence: 1. The diapering surface (changing pad) was ripped and is not longer nonabsorbent.
Plan of Correction:	We will get a new changing pad and make sure we replace them when there are tears or rips.
