1) 60A: A child's record shall contain emergency contact information including the name, address and telephone number of two designated persons to contact in case of an emergency;
2) 520C: Cribs shall have no ore than six centimeters of space between slats; slats must surround all four sides of the crib; a mesh gate is not a substitute for slats;
3) 530B: Cribs when being used by infants shall have a tight-fitting bottom cover;
4) 530C: Linens shall be assigned for individual use;
5) 680B: Caregivers shall ensure that children's hands are washed with liquid soap and warm running water before eating, and after toileting/diapering;

Comments:
An unannounced renewal inspection was conducted from 1:20 - 2:40pm with the provider and her assistant. There were five children in care, (three daycare), ranging from three months to two years of age, for a total of 17 points. Children were observed napping, waking, and playing with age-appropriate toys. One child was observed eating lunch which consisted of macaroni and cheese, corn, peas, pineapple and milk. Three child records and the daily attendance, emergency drill log, menu and required posted information were reviewed. It was reported that there was no medicine on site, and the provider does not have certification in Medication Administration Training (MAT) so cannot administer medication to children in care.

Description: Based on observation, and an interview with the provider, not all children were provided an individual crib for resting/napping. Evidence: There were three infants in care and only one operable crib. There was no crib assigned or available for child #2 (six months old) and child #3 (seven month old).

Description: Based on observation, not all cots in use had linens consisting of a top cover and a bottom cover. Evidence: a toddler was observed sleeping on a cot that did not have a top or bottom cover.

Description: Based on observation, it was determined that not all handwashing procedures were followed, as required. Evidence: A child was observed in a high chair eating and the caregiver did not wash the child?s hands beforehand.
