Comments:
A mandated renewal inspection was conducted on 12/14/2023 from 8:30am-9:20am. There were no children in care with 4 children enrolled 1 staff was preparing the room and getting ready for the children to arrive. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records, 1 staff record and 2 household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-760-A-1
Description: Based on record review, the first aid kit did not have all required items.

Evidence: The first aid kit is missing two triangular bandages.

Plan of Correction: I will get updated immunization records.

Standard #: 8VAC20-800-60-B
Description: Based on record review, the provider did not ensure all required information was in each child's record.

Evidence: Child 1 is missing one emergency contact.

Plan of Correction: I will ask the parents for the information.

Standard #: 8VAC20-800-70-A
Description: Based on record review, the provider did not ensure parents received in writing all required policy and procedures.

Evidence: Child 1 is missing emergency preparedness plan policy, liability insurance declaration, and information to parents document which include check in check out procedures, termination policy, hours of operation, and fees.

Plan of Correction: I will get the policies to the parents.

Standard #: 8VAC20-800-90-C
Description: Based on record review, the family day home did not obtain documentation of additional immunizations for a child.

Evidence: Child 2 does not have documentation of additional immunizations in the record.

Plan of Correction: I will get the immunization records.
