A renewal inspection was initiated and concluded on 12/20/2023. There were 7 children present with the provider and 1 staff supervising. The children were observed during free play time and morning snack time. A total of 5 child records, 2 staff records, and 1 household member were reviewed. The inspection started at 9:20 am and concluded at 10:25 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-A Description: Based on record review and staff interview the center did not ensure to obtain background checks every five years.
Evidence: The most recent criminal history check for staff #1 was dated 12/11/2018 and Household member #3 was dated 12/18/2018.Plan of Correction: Background checks have been completed.
Standard #: 8VAC20-800-90-A Description: Based on record review and staff interview the center did not ensure that before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.
Evidence: The director confirmed that child #1 (start date 12/18/2023) did not have documentation of immunizations.Plan of Correction: Parent was contacted to bring in paperwork.
