Technical Assistance:

Discussed with the provider that each child in care must have their own enrollment record, and that writing siblings' names on the enrollment form is not sufficient. The provider stated that she would ensure to create a separate enrollment record for each child in care. The records can be kept in the same file, but the enrollment records must be completed for each individual child.

Comments:

An unannounced monitoring inspection was conducted on 12/18/2023. There were 10 children (26 points) directly supervised by 3 caregiver(s). The home, 3 caregiver records, 6 children's records, 1 household member's record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely inside the play room. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Description:	Based on review, the provider did not ensure that caregivers and household members sworn statements of disclosure were signed every 5 years from the date of the initial statement. Evidence: reviewed the provider's records and found that sworn statements for Household Member 1 and the provider had expired.

Description:	Based on review, the provider did not ensure that they maintained up-to-date record at the family day home for each enrolled child. Evidence: reviewed 4 children's records and found that there were an additional 3 children enrolled that did not have their own record. Their names and date of birth were merely added to a blank area at the top of the enrollment forms of their older siblings.
