Comments:

An unannounced monitoring inspection was conducted on 12/11/2023 from 10:40am to 12:00pm. There were 6 children ages 10 months to age 4 years old (15 points) with the Provider and 1 assistant. The programming area, attendance, menus, Provider, Assistant and Household member records, 6 children?s records, emergency drills, injury reports, emergency supplies, and policies were reviewed. Children were observed participating in musical instruments, literacy activities, reading, sensory play and singing. Handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on 12/29/23.
