Comments:

An unannounced monitoring inspection was initiated and completed on 12/21/2023. The inspector was on site from 10:30am until 12:30pm. There were 11 children, ages 23 months to 12 years old totaling 15 points under the supervision of one provider. Children were observed during indoor and outdoor free play and lunchtime. Four children's records and one provider records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Description:	REPEAT VIOLATION Based on record review, the center failed to ensure the provider shall obtain documentation of a physical examination by or under the direction of a physician prior to child?s attendance or within 30 days after the first day of attendance. Evidence: Child 3 with an enrollment date of 12/30/2023 had no record of a physical during the inspection on 12/21/2023.

Description:	Based on observation, the provider failed to ensure all areas and furnishing of the family day home, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: In the hallway of the home, there was a wall mirror on the floor. The top of the mirror was broken off exposing the top glass of the mirror.

Description:	Based on observation, the provider failed to ensure the bathroom shall be kept clean and contain a working toilet and sink, toilet tissue, liquid soap and paper towels. Evidence: In the bathroom there was a large bath towel in the floor by the toilet. Provider stated children were using that towel to dry hands.

Description:	Based on observation, the provider failed to ensure that school age children playing outdoors could be heard while caregiver was indoors. Evidence: Upon arrival, Inspector counted 4 school age children outdoors playing. Upon entering the house the caregiver could not hear or see the children outdoors playing.
