Comments:
An unannounced monitoring inspection was conducted on January 3, 2024 between the hours of 9:00am through 10:15am. There were a total of 8 children (16 points) observed with one caregiver. A total of four children's files and three staff/household member's files were reviewed. The children were engaged in the following: toy play, coloring, drawing, and other organized activities. Violations:
Standard #: 8VAC20-800-830-A
Description: Based on documentation review and interview, the family day home did not practice monthly emergency evacuation procedures with all caregivers and children in care and during all shifts that children are in care.

Evidence:
1. Emergency evacuation drills were not practiced the months of November and December.
2. Caregiver #1 confirmed that the emergency evacuation procedures were not practiced for the months of November and December 2023.

Plan of Correction: The provider will conduct two additional emergency evacuation drills to make up for the missing two not conducted in November and December.

Standard #: 8VAC20-800-920
Description: Based on observation, the menu for the current one week period was not dated.

Evidence:
1. The menu for the current one week period was not dated.

Plan of Correction: The provider will date the current menu for the month of January.

Standard #: 8VAC20-800-90-A
Description: Based on record review, the provider did not obtain documentation that a child has been adequately immunized before attending the family day home.

Evidence:
1. Child # 4's record (Start date-09/01/2023) did not have documentation that the child had been adequately immunized. The results from the physical examination from a physician was completed, but the immunization record was blank and did not contain immunization dates.

Plan of Correction: The provider has contacted the parent to obtain a copy of the child's immunization records.
