Comments:
An unannounced monitoring inspection was conducted today from 10:20am-11:10am. There were 12 children directly supervised by 2 caregivers. The child care area, 3 caregiver records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today and there is no caregiver at the family day home who is currently authorized to administer medications.
The provider was not present during the inspection and the caregivers present were supervising children in excess of the supervision guidelines. The caregivers on site did not have access to children or staff files and the provider of another center was called to provide the inspector access to the records.
Children were observed playing freely, napping in cribs and eating snack. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Description: Based on record review, the provider failed to practice shelter-in-place procedures at least twice per year.

Description: Based on record review, the family day home did not keep a written record of children in attendance each day.
Evidence:
At the beginning of the inspection, approximately 10:20am, the attendance sheet for the day had 12 children's names listed and there were 12 children present at the family day home. At 10:36am the attendance document was reviewed and Child 6 and Child 7's names had been blacked out. Child 6 and Child 7 remained in attendance throughout the duration of the inspection.

Description: Based on observation, areas outside the family day home were not maintained in a safe condition.
Evidence:
The turf covering on one of the cement steps leading to the family day home entrance was observed to be ripped at the riser and hanging loosely above the step beneath it, creating a tripping hazard.

Description: Based on observation and record review, the provider failed to ensure that each caregiver did not exceed 16 points by using the following point system: Children from birth through 15 months of age count as four points each; Children from 16 months through 23 months of age count as three points each; Children from two through four years of age count as two points each.

On the date of inspection there were 12 children totaling 39 points under the supervision of 2 caregivers. The caregivers exceeded the point total by 7 points.

Description: Based on observation, infants and toddlers were confined for more than 30 minutes of consecutive time during waking hours.
Evidence:
Child 4 was observed in a bouncy seat at approximately 10:20am and remained there until at least the end of the inspection at 11:10am.
