Comments:

A renewal inspection was conducted on January 3, 2024 from 12:20 P.M.-2:45 P.M. There were five children present for a total of 17 points, ranging in ages from six months to 2 years 6 months, with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, care of children, programming, medication, special care and emergencies and nutrition. A total of four child records, two caregiver records, and four household members background checks were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	8VAC20-800-100-A
Description:	Based on review of children records, the provider failed to ensure that physical examination was in the record prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. Child #2's date of enrollment was 08/09/2023. There was not a physical examination in the file. 2. The provider verified that only page 1 and page 2 completed by the parent were in the record.
Plan of Correction:	Follow-up with the parent to obtain the physical exam. In the future will review all paperwork upon enrollment to ensure all information is obtained and completed.
Standard #:	8VAC20-800-110-B
Description:	Based on review of children record, the immunizations were not signed by a licensed physician, the physician's designee, or an official of a local health department. Evidence: Child #2's date of enrollment was 08/09/2023. The immunizations in the record did not contain a signature, stamp, or electronic signature by a licensed physician, the physicians designee, or an official of a local health department.
Plan of Correction:	Review all paperwork upon enrollment to ensure all required information is complete.
Standard #:	8VAC20-800-170-A
Description:	Based on review of caregiver records, the provider failed to ensure that a tuberculosis screening was completed prior to a caregiver coming into contact with children. Evidence: Date of hire for Caregiver #1 was 11/27/2023. The tuberculosis screening in the record was dated 12/01/2023.
Plan of Correction:	Ensure that all paperwork is completed before employment for future new hires.

Description:	Based on observation, the diaper changing pad was not non-absorbent. Evidence: The diaper changing pad had several rips and tears in the middle and on the side of the pad exposing the foam underneath.
Plan of Correction:	Replaced the diaper changing pad.
