Comments:
An unannounced renewal inspection was conducted on January 5, 2024, from 9:30am-10:30am. At the time of entrance, ( 3) children ( 6 ) points were in care with ( 1 ) caregiver present. The inspector reviewed 3 children?s records and 5 caregiver/household member records. Children were observed playing and engaged in a fine motor activity lacing numbers and letters. No medications were on site. Areas of non-compliance are identified on the violation notice.

The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification of the Acknowledgement Form. Once the background check is received by the provider, the results must be forwarded to the licensing inspector.

Description: Based on a review of household member records, 1 household member has not obtained updated central registry results every 5 years.
Evidence: Household member # 1 has not obtained updated central registry search results. Last results dated 12/27/2018, not within 5 years.

Plan of Correction: The household member will update their central registry search.

Standard #: 22.1-289.036-B-2
Description: Based on a review of household member records, one household member has not obtained fingerprint results once turning 18 years old.
Evidence: Household member # 2 turned 18 years on 12/22/2023. There is no fingerprint results on file.

Plan of Correction: The fingerprints will be scheduled.

Standard #: 8VAC20-770-40-D-5
Description: Based on a review of household member records, one household member has not obtained a search of the central registry once turning 14 years of age.
Evidence: Household member # 3 turned 14 years of age on 8/26/2023, the search for the central registry has not been completed.

Plan of Correction: The household member will obtain their central registry search.

Description: Based on a review of children records, one child's record does not contain the written authorization for emergency medical care.
Evidence: The parent has not signed the written medical authorization for emergency care form for child # 1, date of first attendance 10/1/2023.

Plan of Correction: The emergency care authorization will be signed today at pick up.

Standard #: 8VAC20-800-180-A
Description: Based on a review of household member records, one household member has turned 18 years and has not obtained a current Tuberculosis screening.
Evidence: Household member #2 turned 18 on 12/22/2023 and has not obtained a tuberculosis screening.

Plan of Correction: The household member will obtain a TB screening.
