Comments:

An unannounced monitoring inspection was conducted on 01/05/2024. There were 10 children, ages 3 months ? 4 years, totaling 28 points and 2 caregivers present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 4 children?s records and 2 caregiver records were reviewed. The children were observed during center time. The inspector arrived for the inspection at 10:15am and departed at 11:40am. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Description:	Based on observation and discussion, the provider failed to review the emergency plan at least annually and update the plan as needed. Evidence: The emergency plan was dated 2022 and on the day of the inspection did not have an annual update documented.
Plan of Correction:	Provider will document annual review of emergency plan.

Description:	REPEAT VIOLATION Based on the review of two caregiver records, the provider failed to ensure for assistants and substitute providers documentation that two or more references as to character and reputation as well as competency were checked before employment. Evidence: Caregiver 2 start date was 9/26/2022 and on the day of the inspection did not have two or more references documented prior to employment.
