Comments:
An unannounced monitoring inspection was conducted on January 10, 2024, from 10:30am-11:30am. At the time of entrance, ( 5 ) children (11 ) points were in care with ( 2 ) caregivers present. The inspector reviewed 4 children?s records and 3 caregiver/household member records. Children were observed coloring and playing with toys. No medications were on site. Areas of non-compliance are identified on the violation notice.

The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification of the Acknowledgement Form. Once the background check is received by the center, the results must be forwarded to the licensing inspector.

Standard #: 22.1-289.035-A
Description: Based on a review of caregiver records, 1 caregiver has not obtained fingerprint results every five years as required.
Evidence: Caregiver #1 has not obtained fingerprint results every five years from the last results. The last fingerprint results were dated 8/1/2018.

Plan of Correction: The caregiver will obtain fingerprint appointment and forward the results to the inspector.

Standard #: 22.1-289.035-B-2
Description: Based on a review of caregiver records, 1 caregiver has not submitted fingerprint results prior to hire.
Evidence: Caregiver #2, date of hire 11/30/2023 does not have documentation of fingerprint results. The caregiver was observed caring for children this day.

Plan of Correction: The caregiver will obtain fingerprint appointment and forward the results to the inspector.

Standard #: 22.1-289.036-A
Description: Based on a review of household member records, 1 household member has not obtained fingerprint results every five years as required.
Evidence: Household member #3 has not obtained fingerprint results every five years from the last results. The last fingerprint results were dated 12/20/2018.

Plan of Correction: The household member will obtain fingerprint appointment and forward the results to the inspector.

Standard #: 8VAC20-800-60-B
Description: Based on a review of children records, 2 children records do not have documentation of medical insurance policy number at first day of attendance.
Evidence: Child # 1 and #2, both first day of attendance 11/14/2022 does not have documentation of medical insurance policy number.

Plan of Correction: The provider will obtain the information for the child's records.

Standard #: 8VAC20-800-170-A
Description: Based on a review of caregivers, the provider has failed to obtain from each caregiver at the time of hire a report of tuberculosis screening prior to coming into contact with children.
Evidence: Caregiver #2, date of hire 11/30/2023 does not have documentation of a tuberculosis screening at the time of hire. The caregiver was observed caring for children this day.

Plan of Correction: The caregiver will obtain their TB screening.
