Comments:
An unannounced renewal inspection was conducted today, January 5, 2024. The inspector was on site from approximately 12:30 pm until 2:30 pm. There were two children present with one caregivers supervising for a value of five points. Children were napping upon the inspector?s arrival. Later children were observed having a snack, playing with the kitchen set and dancing to music. The provider interacted with the children in a positive manner. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records, one caregiver and one household member record were reviewed. The inspector interacted with the caregiver and children in care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have any questions regarding this inspection, please contact the licensing inspector.

Violations:
Standard #: 8VAC20-800-700-A
Description: Based on review of medication logs and interview, the provider did not ensure prescription and nonprescription medications were only given to a child with written authorization from the parents.

Evidence:

1. The record of child #5 contained a medication log listing nine dates where a prescription medication was administered to the child. The record did not contain a medication authorization completed by the parent.
2. The provider stated she had administered the medication and the parent had only verbally authorized the administration of the medication.

Plan of Correction: Per the provider: I mis understood the requirement. Going forward I will collect the parents authorization form before administering any medications.

Standard #: 8VAC20-800-730-D
Description: Based on observation, the provider did not ensure medication, including refrigerated medication and medication for caregivers and household members, was kept in a locked place using a safe locking method that prevents access by children.

Evidence:

The providers personal medication was observed on a table in the bedroom. The bedroom door had a lock however the door was open at the time of the inspection. The provider acknowledged the door was open at the time of the inspection. The bedroom is located in an area children are not permitted.

Plan of Correction: Per the provider: The door is always closed and locked. I went in to get some paperwork and forgot to close it behind me. I will make sure it is closed every time after I go in or exit the room.

Standard #: 8VAC20-800-180-A
Description: Based on review of caregiver records, the provider did for each caregiver a current report of tuberculosis screening every two years from the date of the first screening.

Evidence:

The record of caregiver #1 contained documentation of a tuberculosis screening dated 09/22/2021, exceeding two years. The provider stated a current tuberculosis screening had not yet been obtained.

Plan of Correction: Per the provider: An updated tuberculosis screening will be obtained.
