Comments:
An unannounced renewal inspection was conducted on 01/10/24 from 9:00 a.m. to 10:30 a.m. with the Provider. There were three children in care, ranging in age from 23 months to three-years-old, for a total of seven points. Children were observed in free play. Four child records, one staff record, and two household member records were reviewed. The provider has current certification in CPR and First Aid. No medications were on site and the provider understands that they must obtain certification in Medication Administration Training (MAT) in order to give medication. The first aid kit, flashlight, weather band radio, and fire extinguisher were observed. The emergency drill log, attendance, and menu were also reviewed. Areas of non-compliance are identified in the Violation Notice. If you have questions regarding this inspection, please contact the Licensing Inspector, Beth Velke, at beth.velke@doe.virginia.gov or 804-629-8302.

Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 01/17/24.

Violations:

Description: Based on review of documentation; the provider did not implement a semi-annual shelter in place drill.

Evidence: Documentation of the most recent emergency evacuation drill was conducted on 06/27/23.

Plan of Correction: I will do it accordingly, in January and July.

Standard #: 8VAC20-800-240-A
Description: Based on observation the provider did not ensure the family day home was in a clean condition.

Evidence: Food crumbs covered the bottom of the freezer.

Plan of Correction: It was cleaned as soon as the children went home.

Standard #: 8VAC20-800-280
Description: Based on observation, potentially poisonous substances such as disinfectants, were not stored in areas inaccessible to children.

Evidence: A bottle of prescription medication was observed on the stove top near food that was being served to the children.

Plan of Correction: I corrected the error putting them in the upper cabinet.

Standard #: 8VAC20-800-340-C
Description: Based on observation of the bathroom used for children it was determined that not all the required components were available.

Evidence: The bathroom used for children did not have paper towels for drying hands after
handwashing.

Plan of Correction: I put more paper towels as soon as I was told. There were out.

Standard #: 8VAC20-800-540-A
Description: Based on observation the provider did not ensure highchairs were used according to the manufacturer?s instructions.

Evidence: Licensing Inspector observed a 23-month-old child standing in a highchair, an interview with the provider revealed that they had not fastened the safety straps.

Plan of Correction: I will do it all the time.
