Comments:

An unannounced monitoring inspection was conducted on January 2, 2024 from approximately 2:30 pm to 3:30 pm. The inspector reviewed compliance in the areas of administration, personnel, household members, physical environment and equipment, emergencies, caregiver training, preventing the spread of disease and nutrition. The provider record, 3 household member records, and 5 children?s records were reviewed. All areas of the home were observed and areas where the children are cared for were inspected. There were 5 children in attendance for a total of 12 points and 2 caregivers. The children were observed taking a nap and watching a movie while the inspector was on the premises. The first aid kit was inspected and found complete. The menu, emergency numbers, and daily schedule were posted. The information gathered during the inspection determined violations with applicable standards or law. Violations were issued. Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today, 01/16/2024. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Description:	Based on observation, and interview, the provider failed to have a landline telephone available, operable, and accessible during the family day home's hours of operation. Evidence: The provider had a landline telephone available but it was not operable.

Description:	Based on interview, and observation, the provider failed to ensure that cats shall be vaccinated for rabies. Evidence: A cat was observed in the home. The provider acknowledged that the cat lives in the home. The provider was not able to provide the rabies vaccination for the cat.
