An unannounced monitoring inspection was conducted on January 17, 2024 between the hours of 11:15am am through 12:30pm. There were a total of nine children (21 points) observed with three caregivers. A total of five children's files and three staff/household member's files were reviewed. The children were engaged in the following: toy play, puzzles, bathroom breaks, hand washing, lunch,and other organized activities. Areas of non-compliance are identified in the violation notice. Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
Child #3's record (Start date-09/11/2023) did not contain documentation of a physical examination.Plan of Correction: The provider will ask the parents for a copy of the physical examination.
