Comments:
An unannounced monitoring inspection was conducted today from 9:12am-10:00am with the provider on site. There were12 children in care with 3 caregivers. A sample of 2 children's files and 2 caregiver/household files were reviewed. The children were engaged in the following: playing with toys, coloring shapes at the table and sleeping. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and caregivers were positive. The physical space, first aid kit, radio, flashlight, attendance records, drill record and the Emergency Preparedness Plan were reviewed. There were no medications on site. Areas of non-compliance are identified in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver names; list caregivers by positions only.

Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Violations:
Standard #: 8VAC20-800-750-B
Description: Based on observation, a topical skin product was not stored in an area inaccessible to children.
Evidence:
1. An Aquaphor ointment was on the bathroom counter that children use and was accessible to children.

Plan of Correction: Ointments are stored and locked where children can not reach.

Standard #: 8VAC20-800-180-A
Description: Based on interview, the provider did not obtain updated tuberculosis (TB) test or screening for each household every two years.
Evidence:
1. Household Member #3 had a TB test or screening dated 9/14/2021 and had not been updated every two years.

Plan of Correction: Will updated every two years in the future. He is traveling at this time. Will provide TB test as soon as he can be.

Standard #: 8VAC20-800-210-A
Description: Based on interview, one caregiver did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
1. Caregiver #3 (start date 10/04/2022) did not have documentation of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Plan of Correction: This caregiver is fill in only on others absence. I will make sure full time caregivers have 16 hours in future. As time permits.

Standard #: 8VAC20-800-280
Description: Based on observation, potentially poisonous substances such as cleaning agents were stored in an area that was not inaccessible to children.
Evidence:
1. A bottle of Bruce Cleaning Spray, Comet Cleaner and Clorox Wipes were on the kitchen Counter and were accessible to children in the kitchen.

Plan of Correction: All cleaning agents safely locked in the cabinet.

Standard #: 8VAC20-800-320-B
Description: Based on observation, the provider failed to cover all unused electrical outlets accessible to children under five years of age.
Evidence:
1. One unused outlet was not covered in the kitchen, two unused outlets were not covered in the playroom and one unused outlet was not covered by the door under the coats.

Plan of Correction: All electric outlets are covered , plugged securely.
