Comments:
An unannounced monitoring inspection was conducted today between 11:00am and 12:30pm. There were 7 children present with the Provider and 1 assistant for a total of 17 points within the supervision guidelines. The children were observed playing freely with blocks and cars. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations:
Standard #: 8VAC20-800-830-C
Description: Based on interview, documentation of emergency evacuation and shelter-in-place drills was not maintained.
Evidence: There was no evacuation drill documented for the month of December 2023 or a second shelter-in-place drill documented in 2023. The provider stated that the drills were done but they must have forgotten to write them down.

Plan of Correction: I will have to remember to write it down each time.

Standard #: 8VAC20-800-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination prior to each child's attendance or within 30 days of enrollment.
Evidence: Child #4 (date of enrollment: 10/23/2023) record does not contain a physical exam.

Plan of Correction: This week is the child's last week.

Standard #: 8VAC20-800-60-B
Description: Based on record review, children's records did not contain all required information.
Evidence:
1. Child #1s record does not contain an address for their second emergency contact.
2. Child #2s record does not contain the parents contact information or two emergency contacts.
3. Child #3s (date of enrollment: 11/1/2023) record does not contain two emergency contacts or documentation that the child's age and identity has been verified within the first 7 days of enrollment.
4. Child #4s record does not contain two emergency contacts.

Plan of Correction: I will talk to the parents today and ask them to fill in the missing information.

Standard #: 8VAC20-800-480-D
Description: Based on observation, equipment used by children was not used in accordance with the manufacturers instructions.
Evidence: Child #1 was observed sitting in a floor bumbo chair on the providers couch. Manufactures instructions state that the floor chair should be used only on the floor or flat ground.

Plan of Correction: Corrected during the inspection.

Standard #: 8VAC20-800-690-D
Description: Based on interview, the diaper changing surface did not meet all requirements.
Evidence: The diaper changing surface has a soft fabric cover that is absorbent. The provider stated that they wash the diaper changing surface on the weekends. The surface does not get cleaned and sanitized after each use.

Plan of Correction: I will start cleaning after each use.
