Comments:
An unannounced monitoring inspection related to a license renewal was conducted on January 22, 2024 from 2:25 PM --4:28 PM and concluded on January 24, 2024. At the time of entrance, 11 children (age range 21 months to age -10) totaling 19 points were in care with 2 providers/ caregivers. The sample size consisted of 5 children?s records and 2 providers/caregivers records. Children were observed during their rest period, freely playing and preparing for and during their snack time.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-830-A
Description: Based on observation, record review and provider interviews, the provider did not ensure emergency evacuation procedures shall be practiced monthly with all caregivers and children in care.

Evidence: Provider #1 confirmed there was not documentation of drills conducted August, September, October, November and December of 2023.

Plan of Correction: Emergency evacuations have been updated.

Standard #: 8VAC20-800-880
Description: Based on observation and provider interviews, the provider did not ensure when family day homes provide meals or snacks, they shall follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).

Evidence: The provider served only animal crackers and water for afternoon snack and did not meet the requirement for serving 2 out of 5 food groups.

Plan of Correction: Provider will ensue all components are served during all meal times.

Standard #: 8VAC20-800-210-A
Description: Based on record reviews and provider interviews, the provider did not ensue caregivers had obtained in addition to completion of first aid training and CPR training, a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Evidence: Provider #1 had only eight hours of annual training documented and Caregiver #2 had only six hours of annual training. Documented. They did not meet the required sixteen hours annual training.

Plan of Correction: Both providers have al 16 hours of training

Standard #: 8VAC20-800-460-C
Description: Based on observation and provider interviews, the provider did not maintain documentation of the current rabies vaccination.

Evidence: Provider confirmed they had two household cats, and that one cat did not have documentation of obtaining a rabies shot and the second cat?s rabies vaccination expired on 9-27-2023.

Plan of Correction: All shots and vaccinations will be completed by Feb 5, 2024

Standard #: 8VAC20-800-570-A
Description: Based on observation, record review and provider interviews, the provider did not ensure that a caregiver does not exceed 16 points when supervising children.

Evidence: At approximately 3:48 PM one of two providers, Provider #1 left the family day home with 3 children and left the second provider, Caregiver #2 with 8 children that a totaled 17 points. At approximately 4:03 PM Provider #1 returned to the family day home. Caregiver #2 had more than 16 points for approximately 15 minutes.

Plan of Correction: I will ensure that we will stay within our points at all times

Standard #: 8VAC20-800-680-B
Description: Based on observation and provider interviews, the provider did not ensure children?s hands are washed with liquid soap and warm running water before eating.

Evidence: Children were observed preparing for and eating snack and they did not wash their hands prior to eating

Plan of Correction: We will make sure the children wash their hands before and after all meals and snacks
