Comments:
An unannounced monitoring inspection was conducted on 1/23/2024 from 12:20 pm-1:40 pm. There were 6 children (16 points) supervised by 1 direct caregivers. The home, 1 caregivers records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed engaging freely and eating lunch. Areas of non-compliance are identified in the Violation Notice. Please submit your plan of correction within 5 business days for it to be included on the website. Description: Based on record review, the provider did not annually review with the parent the emergency contact information to ensure the information is correct and obtain the parent?s signed acknowledgment of the review.

Evidence:
1. Child #2's last annual emergency contact review was on 1/31/2022.
2. Child #3's last annual emergency contact review was on 7/18/2022.
3. Child #4's last annual emergency contact review was on 7/11/2021.

Evidence: The foot petal on the foot-operated trash can was broken, preventing the trashcan lid from opening on its own.
