Technical Assistance:
1. Reviewed allergy requirements.
2. Inspector will send background check instructions.

An unannounced renewal inspection was conducted at 10:25am. Observed 8 children totaling 25 points with 2 caregivers. Ratios and supervision were in compliance. Children were observed playing with toys, coloring/drawing, washing hands, eating lunch and being diapered. The caregivers were actively engaged with the children and activities. The daycare areas of the home were found to be clean and well supplied with toys and equipment for the children. Areas of non-compliance with standards and laws reviewed were found. Standard #: 22.1-289.035-B-2
Description: Based on records reviewed, the criminal record check on-file for a caregiver was insufficient. Evidence: The criminal record check for Caregiver #3 that was completed in October 2023 was done so with the incorrect legal name.

Plan of Correction: This caregiver is no longer employed in our daycare. For future criminal record checks, we will make sure the correct legal name is provided.

Standard #: 8VAC20-800-780-B
Description: Based on records reviewed, 6 of 9 children's records did not include documentation that parents had reviewed the emergency information annually. Evidence: More than 1 year has passed for Child #1, #2, #3, #4, #8 and #9.

Plan of Correction: These records have now been updated. I will set annual reminders for the parent review checks.

Standard #: 8VAC20-800-120-B
Description: Based on records reviewed, documentation of reference checks were not on-file for a caregiver recently hired. Evidence: There was no documentation of reference checks completed for Caregiver #3 who began employment in October 2023.

Plan of Correction: Since caregiver #3 is no longer employed with us, I will make sure to have reference checks documented for future employees.

Standard #: 8VAC20-800-60-B
Description: Based on records reviewed, 1 of 9 children's records did not include required health/medical information. Evidence: The record for Child #12 did not include actions to take in an emergency situation due to the child's allergies.

Plan of Correction: Child #12 is no longer in our daycare. We will make sure to have emergency action documents from parents for children with allergies for future reference.

Standard #: 8VAC20-800-170-A
Description: Based on records reviewed, 1 of 3 household members have not obtained an initial TB screening. Evidence: There was no documentation of a TB screening for household members #2.

Plan of Correction: Household member #2 turned 18 this past year and is scheduled for a TB test on February 7, 2024. I will make sure TB tests are up to date for future reference.

Standard #: 8VAC20-800-210-A
Description: Based on records reviewed, caregivers did not obtain a minimum of 16 hours of annual training.


1. Caregiver #1 - 13 of 16 hours was documented.
2. Caregiver #2 - 10 of 16 hours was documented.

Plan of Correction: Both caregivers are scheduling for classes to meet the minimum 16 hours of annual training.
