Technical Assistance:
A discussion was held with the caregiver on the requirements for children's records. The inspector left a sample of a complete child's record, provider record and household members record.

Comments:
A monitoring inspection was conducted today between 10:00am and 11:00am. There were 2 children present with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The children were observed coloring. A total of 4 child records and 1 caregiver record were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-750-A
Description: Based on observation, when topical skin products such as diaper ointment and lotion, written parental authorization is required.
Evidence
There are no topical skin product authorizations for diaper creams and lotions. Caregiver #1 confirmed they use topical skin products on child #1, #2, #3 and #4.

Description: Based on review of children's records, not all records contain the required information.
Evidence
The record for child #1 does not contain proof of the child's age and identity, results of the health examination for the child, or parent's signed acknowledgment of the receipt of the emergency preparedness plan.
The record for child #2 does not contain parent's signed acknowledgment of the receipt of the information regarding liability insurance.
The record for child #3 does not contain parent's signed acknowledgment of the receipt of the information regarding liability insurance, proof of child's age and identity, no authorization for the provider to authorize emergency treatment.
