Comments:
A monitoring inspection was conducted on January 10, 2024 with the provider and 2 assistants. There were 8 children present, ranging in ages from 1 year to 3 years, totaling 17 points. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 4 caregiver records were reviewed. The children were having rest time during today's inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on review of four caregiver records, the provider failed to have documentation of a completed search of the central registry (CPS check) on file for each caregiver within the first 30 days of employment. Evidence: Caregiver 2 (date of employment 10/16/2023) did not have documentation of a search of the central registry on file.

Plan of Correction: The provider will check on the status of the CPS check.

Standard #: 8VAC20-800-120-B
Description: Based on review of four caregiver records, the provider failed to obtain documentation of two or more references as to character and reputation as well as competency were checked before employment. Evidence: Caregiver 2 had documentation of one reference. Caregiver 3 didn't have documentation of two references as required.

Plan of Correction: The provider will request references for each caregiver.

Standard #: 8VAC20-800-80-A
Description: Based on review of six children's files, the provider failed to obtain the proof of age and identity for two children. Evidence: Child 3 and Child 7 did not have documentation that the provider that viewed the proof of age and identity.

Plan of Correction: The provider will view proof and identity for each enrolled child within 7 days of enrollment.

Standard #: 8VAC20-800-180-A
Description: Based on review of four caregiver records, the provider failed to ensure caregivers obtained and submitted documentation of a TB test or screening at least every two years from the date of the previous screening. Evidence: Caregiver 1 had a TB test that expired 10/21/2023.

Plan of Correction: The provider will obtain a current TB test or screening.
