Comments:

An unannounced renewal inspection was conducted today from 9:05am-10:00am. There were 4 children directly supervised by 2 staff. The home, 2 staff records, 2 children?s records, household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed reading a story, playing freely and eating breakfast. Diapering procedures were also observed. There was an ample supply of books, toys, and materials for the children. Description:	Based on record review, The provider did not review annually with the parent the emergency contact information to ensue the information is correct. Evidence: Child A ( start date 10/11/2023) did not have annual review of emergency contact information documented in their record.
Plan of Correction:	Model form completed during inspection. We did the orientation we just didn't sign the form.
Standard #:	8VAC20-800-200-A
Description:	Based on record review, the provider did not complete required orientation for assistant by the end of their first week of job responsibilities. Evidence: Staff A - ( hire date 5/1/2023) did not have required staff orientation documented in record by end of first week of job responsibilities.
