A monitoring inspection was initiated and concluded 01/24/2024. The inspector arrived at 10:05 AM and departed at 11:25 PM . There were 6 children present, totaling 13 points, with the provider supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 1 provider/household record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations were reviewed with the provider during the exit interview.

Standard #: 8VAC20-800-100-A Description: Based upon review of children's records, the provider has not ensured that documentation of a physical examination by or under the direction of a physician is obtained prior to a child's attending or within 30 days after the first day of attendance.
Evidence:
The record provided for child 1 indicated that child 1 began attending on 7/27/2023. The record did not include the results of a physical examination.Plan of Correction: The provider responded with the following:
The parent was notified of the needed physical. A deadline will be given if the child is to continue in care.
