Comments:
An unannounced monitoring inspection was conducted on January 22, 2024 from approximately 2:00 pm to 3:00 pm. Provider was present with four children. Children were observed waking up from a nap, eating a snack and being picked up by parents. Four child records were reviewed. Violations were found and are noted on the violation notice.

Standard #: 8VAC20-800-920
Description: Based on observation and interview, provider did not ensure when meals or snacks are provided there is a menu posted for the current week showing what is served.

Evidence: A menu for the current week was not posted. Provider confirmed she provides meals/snacks and that the menu has not been printed.

Description: Based on observation and interview, provider did not ensure there is a written record of children in attendance each day.

Evidence: Provider stated she did not have a written attendance of children today. Four children were observed present.

Based on review and interview, provider did not ensure 2 of 4 child records have documentation of required information.

Evidence: The following was found during record review for children present during inspection:
1. The record for child #1 did not have documentation of start date, father's home information, parent's employment information, emergency contact information, physician information and signed written agreement with parents.
2. The record for child #4 did not have documentation of start date.
3. Provider confirmed information was not available during inspection.

Description: Based on observation, provider did not ensure areas and furnishings of the home are maintained in a clean, safe and operable condition to include unstable heavy items that a child could pull down on himself.

Evidence: A 6' step ladder was closed and propped against the wall in the hallway immediately outside the bathroom used by children.

Evidence: Two multi-purpose cleaners labelled "Caution and keep out of reach of children" were observed in an unlocked cabinet under the sink in the bathroom used by children.

Description: Based on observation, provider did not ensure the temperature of all inside areas occupied by children shall be maintained no lower than 65 degrees.

Evidence: The temperature measured 61 degrees in the childcare room at 2:34 pm during inspection.
