Technical Assistance:
Discussed that the shelter in place drill records should be written on their own line and not listed on the same line as the fire drills. Discussed the menu needs more detail and specifically list what type of fruit is given, what type of vegetable given, etc. Example-Menu had Fruits listed-it should list what type of fruit, Menu had vegetables listed-it should list what type of vegetable.Discussed the cribs will need documentation of their manufacture date or will need to be on the crib to view.

Comments:
An unannounced monitoring inspection was conducted today from 11:27am-12:45pm with the provider on site. There were 10 children present with 3 caregivers. A sample of 2 children's files and 3 caregiver/household files were reviewed. The children were engaged in the following: playing with toys and one child was eating. The site was clean. Interactions between the children and caregivers were positive. The physical space, attendance records, drill record, first aid kit and the Emergency Preparedness Plan were reviewed. There were no medications on site. Areas of non-compliance are identified in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver names; list caregivers by positions only.

Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Standard #: 8VAC20-800-750-B
Description: Based on observation, topical skin products were not kept inaccessible to children.
Evidence:
1. Three diaper ointments were inside bin drawers in the bathroom children use and were not kept inaccessible.

Plan of Correction: We will put the ointments in an inaccessible location.

Standard #: 8VAC20-800-790
Description: Based on observation, the responsible person for emergency back up care was not posted in a visible area close to the telephone with all required information.
Evidence:
1. The provider had the emergency back up care person name and phone number listed on the parent bulletin board, but did not have the emergency back up care person's address and was not near the telephone.

Plan of Correction: We will be putting the address underneath the telephone number on the bulletin board.

Description: Based on review of the menu, meals and snacks are provided by the family day home and the menu for the current one week period did not include all requirements..
Evidence:
1. The provider had a menu that listed food items, but was not specific of what kind of food was served. For example-it listed fruit and veggie, but did not list what type of fruit or veggie.

Plan of Correction: While we inform the parents everyday what the children eat, we will provide more details on the menu.

Standard #: 8VAC20-800-120-B
Description: Based on review of records, the provider did not maintain complete assistants' records.
Evidence:
1. Assistant #2's file was missing the following: name, address, emergency contact name, address, telephone number and date of employment.

Plan of Correction: We will include the missing information in the caregivers folder.

Standard #: 8VAC20-800-50-C
Description: Repeat Violation
Based on observation, information contained in a child's record was not kept privileged and
confidential.
Evidence:
1. The supplemental page from the inspection dated 8/08/2023 was posted on the wall. This is considered confidential.

Plan of Correction: Noted, we will remove the supplemental page.

Standard #: 8VAC20-800-180-A
Description: Repeat Violation
Based on documentation review, the provider did not obtain updated tuberculosis (TB) test
for each caregiver every two years.
Evidence:
1. Caregiver #2 (start date 9/08/2021) had a TB test or screening dated 9/09/2021 and was expired.

Plan of Correction: Caregiver #2 will be getting her TB test performed again.

Standard #: 8VAC20-800-200-C
Description: Based on interview and review of documentation, one assistant did not have documentation of the orientation signed and dated by the provider and substitute provider or by the provider and assistant.
Evidence:
1. Assistant #3 (unsure of start date) is employed by the provider as an assistant and their file did not have documentation of the orientation. The provider stated she did give the orientation training.

Plan of Correction: We will include the documentation in her folder.

Standard #: 8VAC20-800-240-A
Description: Based on observation, furnishings of the family day home inside were not maintained in a safe and operable condition.
Evidence:
1. One crib in the kitchen area on the basement level had two large cracks (each crack was approximately 6 inches long) on the base of the crib under the mattress. There was also one loose screw under the mattress.

Plan of Correction: We will repair the crack and [tighenting] the screw.

Standard #: 8VAC20-800-280
Description: Repeat Violation
Based on observation, potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents were stored in areas that was not inaccessible to children.
Evidence:
1. In the bathroom children use, a Lysol multipurpose cleaning product was under the sink in the bathroom cabinet and was not out of reach of children and was not kept locked under the sink.

Plan of Correction: We will be putting a cabinet lock.

Standard #: 8VAC20-800-520-C
Description: Based on observation, 8 out of 12 cribs did not meet the crib requirements.
Evidence:
1. 8 cribs did not have the manufacture date or documentation to document the cribs meet the US Consumer Safety Commission standards at the time they were manufactured.

Plan of Correction: We will locate a copy of the crib documents.
