Comments:
An unannounced monitoring inspection was conducted on January 31, 2024 between the hours of 9:40 am through 10:45am. There were a total of four children (14pts points) observed with one caregiver. A total of four children's files and two staff/household member's files were reviewed. The children were engaged in the following: t breakfast, toy play and other organized activities. Violations:
Standard #: 8VAC20-800-700-A
Description: Based on record review and interview, the provider did not have written authorization from the child's physician and parent for a long term prescription medication.
Evidence:
1. Child #4 has a long term prescription medication on site without a medication authorization form completed and signed by the parent and the child's physician.
2. Caregiver #1 confirmed that a medication authorization form was not completed by the parent and physician at the time of the inspection.

Plan of Correction: The caregiver will ensure that the parent receives the proper documentation for the long term medication and signed by the parent and physician.
