Standard #: 8VAC20-800-700-A Description: Based on record review and interview, the provider did not have written authorization from the child's physician and parent for a long term prescription medication.
Evidence:
1. Child #4 has a long term prescription medication on site without a medication authorization form completed and signed by the parent and the child's physician.
2. Caregiver #1 confirmed that a medication authorization form was not completed by the parent and physician at the time of the inspection.Plan of Correction: The caregiver will ensure that the parent receives the proper documentation for the long term medication and signed by the parent and physician.
