Technical Assistance:
Forms for children's records, fireplace inspection, maintenance of space to avoid trip and injury hazards, capacity and points, small appliances.

Comments:
An unannounced renewal inspection was conducted on 2/1/24 from 2:30pm to 3:35pm. There were 8 children (16 points) in care of the provider. The children were observed in free play, handwashing before PM snack and eating snack. The snack included veggie chips and cookies. The family day was inspected to assess for compliance. Two children's records, 1 caregiver's record, emergency drill logs, attendance records and other daycare postings were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-800-100-A
Description: Repeat Violation
Based on records review, the provider did not obtain documentation of a physical examination for a child enrolled within 30 days after the first day of attendance.
Evidence:
1. Child # 1's first day of attendance was 9/11/23.
2. Documentation of a physical exam has not been obtained for Child # 1 to this date.

Plan of Correction: The physical will be obtained from the parent and kept on file.

Standard #: 8VAC20-800-40-A
Description: Based on observation, the total number of children receiving care exceeded the maximum licensed capacity of the home.
Evidence:
1. The family day home is licensed for a maximum capacity of 7 children.
2. At 2:30 pm, there were 8 children in care of the provider.
3. The provider was over capacity until 3:24 pm when one child left.

Plan of Correction: The child's parent needed care. I will ensure that this does not happen again.

Standard #: 8VAC20-800-60-B
Description: Repeat Violation
Based on records review, children's record did not have complete information.
Evidence: Child # 1's record was missing the following information: names of persons authorized to pick up, complete address for one emergency contact, authorization for emergency medical care and documentation of the proof of identity and age of the child.

Plan of Correction: All missing information will be collected and the paperwork will be updated.

Standard #: 8VAC20-800-280
Description: Repeat Violation
Based on observation, hazardous materials such as cleaning agents were accessible to children.
Evidence:
1. The children were observed playing in the basement next to the laundry room.
2. The laundry room door was open. There were bottles of cleaning supplies in the laundry room which were accessible to the children.

Plan of Correction: The door to the laundry room was locked during the inspection. It will remain locked whenever children are in care.
