Comments:
An unannounced on-site monitoring inspection was conducted on 01/22/2024. There were 4 children, ages 2 years-3 years, and the provider at a total 6 points. The inspector observed the following: nap time. A total of 2 children?s records and 1staff record and a household member records, were reviewed. The inspector arrived at 12:30 PM and departed at 1:50 PM.

Standard #: 8VAC20-800-700-C
Description: Based on review of medications and authorizations, the center failed to ensure that all medications were returned to the parent or guardian within 14 days after the authorization expired or disposed of by the center.

Evidence:

Three medication authorizations for Child # 3 expired 14 days after 09/21/2023. As 01/22/2024, the medication had not been returned to the parent or disposed of by the center.

Plan of Correction: Provider will return medication to parent, and will return when an authorization is completed by the doctor.

Standard #: 8VAC20-800-180-A
Description: Based on of two records, the provider failed to ensure that all repeated TB screening or test were completed every two years.

Evidence:

1. Household member # 1 most recent TB screening was dated 01/01/2022.

Plan of Correction: The provider will send the household member for a TB and monitor all dates.
