Comments:

An unannounced renewal inspection was conducted February 5, 2024 between 9:25am and 10:50am. There were 3 children present with one (1) caregiver supervising within the supervision guidelines. The children were observed playing with various age appropriate toys, handwashing, eating snack, and bottle feeding. Positive interactions were observed during this inspection. The inspector reviewed compliance in the areas of administration, required posted documents, physical plant, staffing and supervision, programing, evacuation drill logs, menu, first aid supplies, attendance, child and caregiver records, and emergency procedures. No medication was on site at this time. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	22.1-289.036-A
Description:	Based on record review, the provider did not obtain documentation for a 5 year repeat check. Evidence: 1. The record for household member #1 did not have documentation of a 5 year criminal history repeat check. 2. The last documentation of a criminal history check expired on 10/3/23.
Plan of Correction:	I will resubmit tonight. I didn't realize I submitted the wrong one. The provider submitted the portability on 1/4/24.
Standard #:	8VAC20-800-780-B
Description:	Based on record review, 1 of 3 children files indicated that annually, the provider did not review with the parent the emergency contact information required in 22 VAC 40-111-60 B 2 to ensure the information is correct, and obtain the parent's signed acknowledgment of the review. Evidence: The record for Child #2 did not have documentation that the provider had reviewed the emergency contact information annually with the parent.
Plan of Correction:	I will be sure to have parents review yearly. The parent came to pick up their child and reviewed the paperwork during the inspection. Violation was corrected during the inspection.
Standard #:	8VAC20-800-100-A
Description:	Based on record review, the provider did not obtain documentation of a physical examination within 30 days after the first day of attendance. Evidence: The record for Child #3 did not have documentation of a physical examination. The date of enrollment is 11/27/23.
Plan of Correction:	I will tell my parents they must bring the physical within 30 days of starting.
Standard #:	8VAC20-800-90-A
Description:	Based on record review, the provider did not obtain documentation of immunization before a child attends the family day home. Evidence: 1. Child #3 started on 11/27/23 and the immunization record is dated 02/05/24. Repeat Violation
