Comments:
An unannounced renewal inspection was conducted today from 10:30am-11:40am. There were 11 children (totaling 22 points) directly supervised by 2 caregivers. The home, 2 caregiver records, 3 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and eating snack. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-770-40-D-4
Description: REPEAT VIOLATION
Based on review of 3 household member records, the family day home failed to obtain sworn statement when a person in the home became 18 years old.
Evidence
1. Household member (HHM) #2 turned 18 December 2022, HHM#2 did not have a sworn statement on file.
2. Caregiver #1 confirmed that HHM#2 had not completed sworn statement.

Plan of Correction: I sent it to my Inspector.

Standard #: 8VAC20-800-760-A-1
Description: Based on observation and caregiver interview, the family day home did not have all emergency supplies in the family day home.
Evidence: The first aid kit in the family day home did not contain adhesive tape, antiseptic cleaning solution or pad, or a First Aid instructional manual.

Plan of Correction: I get all the necessary, missing supplies from my car, and put it in my First Aid Kit.

Standard #: 8VAC20-800-770
Description: Based on observation and caregiver interview, a working portable battery-operated weather
band radio, were not available to caregivers at all times.
Evidence: Caregiver #1 confirmed that the home did have a portable battery-operated weather
band radio.

Plan of Correction: We will buy a weather band radio asap.

Standard #: 8VAC20-800-780-B
Description: Based on record review, the provider did not review with the parent the emergency contact information required to ensure the information is correct, and obtain the parent's signed acknowledgment of the review, annually.
Evidence:
1. Child #1 (start date 9/15/22) most recent emergency contact information was dated 9/13/2022
2. Child #3 (start date 9/1822) most recent emergency contact information was dated 9/8/2022

Plan of Correction: I got it corrected today with parents.

Standard #: 8VAC20-800-890
Description: Based on observation, when food was brought from home , the food container was not clearly labeled in a way that identified the owner
Evidence: Child#1 and child #4 had food containers that were not labeled with their name.

Plan of Correction: We labeled everything in front of inspector during the inspection.

Standard #: 8VAC20-800-60-B
Description: Based on record review, the provider did not maintain an up-to-date record at the family day home for one of three children.
Evidence: Child #1's(start date 9/15/22) file did not contain name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: I checked and updated all my kids records, and made parents sign them.

Standard #: 8VAC20-800-230-B
Description: Based on record review and caregiver interview, caregivers did not obtain a minimum of sixteen clock hours of training annually.
Evidence: Caregiver #1 confirmed that Caregiver #1 and caregiver #2 did not complete any training in 2023.

Plan of Correction: We will complete training asap.
