An unannounced monitoring inspection was conducted on February 5, 2024 from approximately 1:15 pm to 2:35 pm. Two caregivers were observed supervising nine preschool age children. Children were observed napping, eating a afternoon snack and playing outside. Four child and three caregiver records were reviewed. Violations found are noted on the violation notice.

Standard #: 8VAC20-800-920 Description: Based on review, provider did not ensure when meals or snacks are provided, the menu for the current week is posted.
Evidence: The menu posted was dated for January. Children were served apples and cheese crackers for snack and it was not on the menu posted.Plan of Correction: Not available online. Contact Inspector for more information.

Evidence: The following was found during record review:
1. The records for child #1 and child #2 did not have documentation of start date and signed acknowledgement of parent's notification of emergency preparedness plan.
2. The record for child #3 (start date 8/21/2023) did not have documentation of a second emergency contact, immunization record and signed acknowledgement of parent's notification of emergency preparedness plan.
