Comments:

A monitoring inspection was initiated on 02/08/2024. 10 children (22pts) were present with 2 caregivers. The physical space, evacuation drills, safety supplies, attendance records, caregiver records, 2 child ren's records and required posted documents were reviewed. Children were observed playing with developmentally appropriate materials, interacting with each other and engaging with the caregivers. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued. Standard #:	8VAC20-800-100-A
Description:	Based on records reviewed, 1 out of 2 children's records did not contain a physical within 30 days after the first day of attendance. Evidence: Child A has been in attendance for over 30 days and did not have a physical in their file.
Plan of Correction:	I will ask for the physical right away.
Standard #:	8VAC20-800-180-A
Description:	Based on record review, subsequent tuberculosis screening forms were not completed every two years for a caregiver and household member. Evidence: The previous tuberculosis screening forms, for Caregiver A and Household Member A, both expired on 12/03/2023.
Plan of Correction:	We will get the TB screenings completed right away.
